Physical Therapist- San Antonio, Texas

OVERVIEW:  Working with us means one thing: it’s all about the patients. We’re seeking a certified Physical Therapist (PT) with a passion to work with orthopedic and/or neurologic patients in an out-patient, high paced environment.

What will be required from you:

  • Evaluates and administers treatment in consultation with the referring physician
  • Completes all appropriate documentation in the patient’s medical records
  • Instructs patient and their family in appropriate patient care
  • Assists in total rehabilitation of the patient
  • Assists in training support personnel in the provision of in-service programs

 What you’ll need to be successful:

  • MUST BE: Physical Therapist - Certified (Must be a graduate or soon-to-be graduate of an accredited physical therapy program)
  • MUST HAVE: Manual Therapy Experience
  • MUST HAVE at least one (1) year Outpatient Orthopedic experience
  • Minimum Bachelor’s degree
  • Ability to work in high paced environment
  • Certified Orthopedic Manual Therapist (COMT) Cert a plus!
  • Orthopedics Specialist Certification (OCS) Cert another plus!

Case Manager- Los Angeles, CA

Seeking a Case Manager to provide ongoing services and support in the community to homeless/formerly homeless veterans enrolled in the HUD VASH (Veterans Affairs Supportive Housing) Case Management Program presenting with complex treatment and housing needs.

 

Working as part of a team of dedicated and supportive professionals, the HUD-VASH Case Manager provides services to high needs homeless/formerly homeless Veterans with active mental illness, substance abuse disorders, and co-occurring medical conditions in the community and their homes.

 

Flexible work schedule with minimal requirements to report to office. Supportive management; free supervision towards licensing (LCSW and LMFT). Comprehensive benefit package including medical, dental, vision, life insurance, 401(k), PTO and sick leave. This position offers a highly competitive starting salary and will give the employee the opportunity to master the VA system, our nation’s largest provider of health services to our veterans.

QUALIFICATIONS:

  • Master’s degree in the social sciences from an academic institution accredited in Social Work and/or Psychology. (e.g. Social Work, Mental Health Counseling, Psychology, others as appropriate)
  • Minimum of two (2) years’ experience providing case management. Prior experience working in clinical and or housing/supportive housing programs strongly preferred.
  • Experience working with Veterans, homeless populations, and/or people with active co-occurring disorders.

WORK ENVIRONMENT:

This is a field-based position involving substantial time in the community inclusive of private homes, the shelter system, hospitals, and other public settings.

 

MISC:

  • All employment offers are contingent upon successful completion of a Security/background investigation via NACI (National Agency Check with Written Inquiries) and a SAC (Special Agreement Check) which includes electronic fingerprinting and drug screening.

RN Care Manager- Brooklyn, NY

Substance Abuse Counselor- Los Angeles, CA

The primary role of the HUD-VASH Substance Abuse Counselor is to provide ongoing substance use case management and early treatment interventions to promote sustained stability in Veterans enrolled in the HUD-VASH program.

All services are grounded in a Housing First philosophy and are designed to help enrolled veterans referred by the U.S. Department of Veterans Affairs secure and maintain stable housing and prevent a return to homelessness. Core daily activities include outreach, service coordination, home visitation, and linkage to medical/behavioral health/substance abuse treatment/housing services/public benefits.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Develop appropriate professional relationships with the Veterans and meet the Veteran where he or she is ready to engage in treatment.
  • Develop liaison relationships with more acute treatment providers/programs, such as an Intensive Outpatient Program.
  • Provide appropriate group and individual case management and treatment.
  • Provide Outreach services for HUD-VASH Veteran clients in the community to Substance Abuse treatment programs and providers.
  • Provide training for Clinical and Case Managers in areas related to substance use disorder (SUD) assessment, anger management, life skills and co-occurring disorders.
  • Conduct intake and needs assessment of homeless Veterans and identify symptoms leading to clinical diagnoses of SUD.
  • Provide assessment functions, e. g. interviewing, psychosocial histories, and SUD assessments to aid in the development of recovery/treatment plans.
  • Develop comprehensive individualized recovery/treatment plans with the Veteran that address the factors that contributed to homelessness, including SUD and other co-occurring factors within a recovery model framework.
  • Work with the Veterans to realize the Veteran's treatment goals and reassess and revise goals as indicated by the Veteran's engagement in treatment.
  • Organize and conduct individual and group case management and SUD treatment, to include early interventions and supportive follow-up to ensure stability.
  • Accurately and expediently document all services rendered in an electronic health record system.
  • Handle confidential material in a responsible manner.
  • Attend staff meetings, case conferences, group and individual supervision and trainings as scheduled.
  • Handle unexpected situations by responding effectively to challenges, deadlines, and emergencies; cope adequately with stress on the job.
  • Provide culturally competent services in accordance with Company policies.
  • Performs other duties as assigned.

QUALIFICATIONS:

  • Certified Substance Abuse counselor with a minimum of a Bachelor’s degree from and accredited institution and one (1) year of demonstrated experience providing substance abuse counseling.
  • Current, active, unrestrictive Certified Alcohol Drug Counselor Associate (CADCA) certification from the California Commission for Behavioral Health Certifications.
  • Experience working with Veterans, homeless populations, and/or people with active mental illness/co-occurring behavioral health and or substance abuse disorders preferred.
  • Prior experience working in clinical and/or housing/supportive housing programs strongly preferred.
  • Flexible work schedule to include some evenings and/or weekends as needed.
  • Possesses a high level of tolerance and understanding for individuals who present for services with urgent multiple case management and health needs.
  • Strong written and verbal communication skills.
  • Computer literate, MS/word and Excel.
